This rule requires each licensed entity under Alabama's Department of Mental Health to maintain and make available for onsite review key governance documents, including board-approved operational policies, articles of incorporation or charter, bylaws, a current organizational chart, a written mission statement approved by the Board of Directors, written board responsibilities, and minutes of board meetings. These are administrative governance requirements applicable to the facility types licensed under Chapter 580-3-25.
