This regulation requires inpatient psychiatric facilities to notify parents, guardians, or persons in loco parentis when a minor aged 14–18 is voluntarily admitted for treatment, using both telephone and Form MH-781. The notice must explain the proposed treatment and the parent's right to object. If the facility cannot locate the parent or guardian, the director must take appropriate action, including contacting child welfare agencies. Parents who object may file a written objection and request a hearing.
