2103 APPLICATION PROCEDURES

2103.1 Loan application forms may be obtained by writing to the Commission of Public Health, Office of Health Planning and Development, at 1660 L Street, N.W., Washington, D.C. 20036.

2103.2 The prospective borrower shall complete a loan application form and submit it to the Office of Health Planning and Development.

2103.3 The prospective borrower shall provide documentation of the purpose of the loan as described in §2103.5.

2103.4 The group or entity requesting the loan shall provide written proof of the stated purpose of the loan to the loan review committee.

2103.5 The proof shall be in the following form:

(a) A lease agreement from the lessor;

(b) Repair or renovation estimates from a reputable firm; or

(c) A purchase order receipt from an established business entity for allowable goods as defined in §2102.3.

2103.6 Three (3) estimates or price quotes shall be required for items other than a lease agreement for which the loan is requested.
