This section establishes requirements for assessment and individual treatment plans (ITPs) for Evidence-Based Treatment (EBT) services delivered to children, youth, and young adults. Operators must complete a face-to-face assessment within 15 days of service initiation and prior to completing the ITP, and must review and update both the assessment and ITP in accordance with the applicable cross-referenced sections of Chapter 5240.
