This regulation requires licensed substance use residential treatment facilities to develop individualized treatment and rehabilitation plans collaboratively with each client, documenting short- and long-term goals, service types and frequencies, and proposed support services. Plans must be reviewed and updated at least every 30 days. Counseling services—including individual and group counseling at least twice weekly, plus family and couple counseling as appropriate—must be provided and documented per the plan. The facility must also assist clients in accessing supportive services such as medical, psychiatric, legal, economic, educational, vocational, and recreational resources.

This plan shall include, but not be limited to, written documentation of: (1) Short- and long-term goals for treatment as formulated by both staff and client. (2) Type and frequency of treatment and rehabilitation services. (3) Proposed type of support service. (b) Treatment and rehabilitation plans shall be reviewed and updated at least every 30 days. (c) The project shall assure that counseling services are provided according to the individual treatment and rehabilitation plan. (d) Counseling shall be provided to a client on a regular and scheduled basis. The following services shall be included and documented: (1) Individual counseling, at least twice weekly. (2) Group counseling, at least twice weekly. (3) Family counseling, as appropriate. (4) Couple counseling, as appropriate. (e) The project shall assist the client in obtaining the following supportive services when necessary: (1) Medical/dental. (2) Psychiatric. (3) Legal. (4) Economic. (5) Educational. (6) Vocational.
