(1) The provider shall involve the child and the child’s parent or guardian to the fullest extent possible at all stages of treatment planning and discharge planning toward the goal of reintegrating the child into the community. (2) The child’s discharge plan shall be reviewed and, if necessary, revised during each review of the treatment plan. (3) The provider shall design individualized services and treatment for the child to address the child’s presenting problems on admission with a goal of discharge to the community or to a step-down program within 120 days of admission for residential treatment centers and 365 days for therapeutic group homes.
